Support at Home is an Australian Government-funded aged care program designed to help older people live safely, independently, and with dignity in their own homes through personalised care, practical assistance, equipment, and home modifications tailored to their assessed needs.

What does Support at Home provide?

Under Support at Home, you have access to a defined list of services to support you to live at home for longer.

Clinical Support

Professional nursing care, medication management, and health monitoring delivered by our qualified clinical team.

Expert Care

Independence Support

Assistance with mobility, transport to appointments, and allied health services to keep you active and engaged.

Stay Active

Everyday Living

Helping with cleaning, meal preparation, shopping, and personal care tasks to keep your home running smoothly.

Daily Comfort

It also includes services that support culturally safe care, such as help to join in cultural activities and access to Aboriginal and Torres Strait Islander health practitioners and health workers.

How much Funding do I receive?

Each participant receives one of 8 funding classifications for ongoing services. Their classification is determined in their aged care assessment.

You will work with participants to spend this amount on care and services in the Support at Home service list, according to their assessed needs.
